In 2019, Community Gateway introduced a Nursing Program to provide holistic health care and support for the people we work with. The aim of this Program is to ensure we give our clients the best possible chance of remaining in their home for as long as possible. We do this by anticipating their needs and working with them to ensure they get the outcomes relevant to them. 

As part of our Nursing Program, all Home Care Package (HCP) clients receive a clinical health assessment at least once per year. For some clients this may be on a more regular basis, depending on the complexity of their health issues. Our Nurses work closely with the Care Managers and Care Workers to ensure our clients are receiving the highest level of care at every opportunity.  

We have employed Registered Nurses across all three of our locations - the Illawarra, Shoalhaven and Kempsey who are qualified to assist clients with the following:

  • Health Assessments 

  • Nutritional Support 

  • Diabetic Support (as per a Diabetic Management Plan)

  • Continence Supports (such as catheter care and maintenance) 

  • Wound, Pressure and Skin care (e.g. changing dressings)
