About the role

The Mentor will provide exceptional quality, person centred, strengths based, outcome focused service delivery within the settings of workshops and client action plans to support people to achieve their true potential.

The Mentor plays a pivotal role in delivering Community Gateway’s ‘Why’ Everything We Are Is Relevant To You, in all interactions with clients and staff members

Success in this role will be representative through leading the service delivery to achieve:

  • positive client outcomes,

  • increased health & wellness,

  • and goal achievement.

 

Achieved through an innovative, dynamic environment focussed on client strengths, and person-centred service delivery.

 

About you

You are a caring and enthusiastic individual, who is ready to bring your experience to help our clients! We are after a person who has a passion to help others, by making sure everything they do is the most relevant to our clients.

  • Understanding and experience in delivering person centred inclusive supports for people ageing and/ or with a disability

  • Understanding of, and commitment to delivering person-centred, holistic health, wellness, reablement and strengths-based services

  • Experience in outcome focused service delivery

  • Ability to mentor to support autonomy, independence, and task achievement

  • Ability to contribute and support a positive team environment

  • Experience in networking, building community connections and promoting inclusion

  • Excellent communication and timely reporting and record keeping skills

  • Ability to follow policies and procedures and implement quality processes

  • Demonstrated understanding of and commitment to work, health, and safety requirements

 

Desirable:

  • Certificate III qualification in Individual Support

  • 3 years demonstrated experience in delivering outcome focused services

 

The successful applicant will need (or be willing to obtain):

  • Current First Aid and CPR Certificate

  • Current Driver's Licence and access to a reliable and insured motor vehicle

  • Availability to a Smart Phone.

  • Working with Children Check

  • NDIS Worker Screening Check

  • Proof of vaccination COVID 19
